Bring over unmerged PRs from ghcide

This issue exists to track progress on the following PRs that were open on ghcide but not merged:

• Update for haskell-lsp-1.0.0.0 ghcide#857 (Upgrade to lsp-1.0 #1284)
• Locate Documentation html page using Haddock interface file ghcide#861
• Do not suggest any hole filling if there are too many suggestions ghcide#888
• Use hole fit plugins to record holes instead of parsing error messages ghcide#889: tracked in its own issue Don't suggest hole filling for ambiguous holes or when there are too many suggestions #532
• Integrate hiedb ghcide#898 (References via hiedb #704)
• Clarify recompilation logic ghcide#891 (not correct)
• Add code actions for disabling a warning in the current file ghcide#897 (Add code actions for disabling a warning in the current file #1235)

Happy to ignore 888 for now, it can be worked around with GHC flags anyway.

Yes, I actually got half way to reopening haskell/ghcide#897 last night - just need to fix up a test on older GHCs.

let's ignore the haskell/ghcide#861 for now, as I'm blocked by upstream haddock anyway for indefinite time. By the time when it's fixed (if ever) hls sources may drift a lot.

btw, if I can fix and publish the haddock upstream myself I'd be happy to do so, but it seems to lack owners, at least I had no response on the issues reported
