Link topic page(s) in the rendered topic header

As a followup to #2579 implementing #2572 , it would be very useful to have the `Topic` header in the rendered PEPs actually link to said topic(s), and would be easy to implement (so long as we keep the display names consistent with the URLs, minus caps/whitespace).
